The Reach for the Stars Case Management team works with individuals who have been diagnosed with intellectual or developmental disabilities in order to assess, coordinate, and connect them with appropriate services to meet their needs and desires. We assist individuals with their application for Kentucky Medicaid Waiver programs that meet their needs.

The mission of Reach for the Stars Case Management is to provide high quality, individualized case management services that support the person to achieve reams and goals that are limitless as the stars.
Reach for the Stars Case Management offers more than 70 years of combined experience in the field of Human Services. We provide quality case management services using person centered planning.

Reach for the Stars will work specifically with those in the state of Kentucky who have a diagnosis of intellectual disorder and/or developmental disability.
Sources of funding are provided through the Supports for Community Living, Michelle P. Waiver, and Kentucky Transitions programs.
